Pregnancy booking and maternity self-referral 🤰

If you are newly pregnant, you do not need to visit your GP surgery to access maternity care. You can refer yourself directly using the Badger Notes self-referral form.

How to refer yourself

To access maternity services, please complete the Badger Notes self-referral form online.

What happens next?

Step 1: Complete your details on the online booking form.

Step 2: You will receive an email confirming your form has been received safely.

Step 3: Within 1 to 2 weeks, a community midwife will contact you to arrange an appointment.
