The Bay Health Local Cluster Collaborative is one of eight Local Cluster Collaboratives in the Swansea Bay University Health Board Area.
There are eight practices which operate within Bay Health Local Cluster Collaborative covering Uplands, Sketty, West Cross, Mumbles, Killay and Gower, also serving students and residents at Swansea University.

About the Cluster
Bay Health Local Cluster Collaborative brings together partners across primary care including GP practices, community nursing, optometry, dentistry, community pharmacy, allied health professionals and Voluntary Sector services.
The Bay Health Local Cluster Collaborative is the largest within Swansea Bay and cares for approximately 75,000 people, including a diverse student population and a large proportion of older residents.
Bay Health Local Cluster Collaborative aims to:
Prevent ill health, enabling people to remain well and independent for as long as possible. The cluster commissions wellbeing services accessed through social prescribers. It employs multidisciplinary professionals including chronic condition nurses, cluster pharmacists who perform medication reviews, and a cluster business manager.
The cluster works to develop and coordinate high-quality community services across health and social care partners. This collaborative approach strengthens communication and information sharing and helps link community and hospital services, including through initiatives such as the Swansea Bay Virtual Ward.
