You must have a blood form to book your appointment for a blood test.

• Please note that this service is for patients 12 years or older.
• If a blood test is required for a patient under 12 years old, please contact the Paediatric Clinic on 01792 530636.
• We are unable to see patients under 12 in any of our out patient departments.
