If you change your address you will need to notify the Practice as soon as possible. In order to comply with the Data Protection Act notification of any changes need to be in writing. Please see the Online services which will take you to a link to electronically send the information to the practice alternatively ask at reception for a notification form for you to complete.
Remember to also notify the hospital if you are awaiting an appointment or already undergoing treatment.
If you move outside our defined Practice Area, which is outlined on the back of the patient information pack, it is important that you register with another doctor in that area as soon as possible to avoid any problems should you need a doctor urgently. The doctors are responsible for patients at the address shown on their medical records held in the Practice.
My Health Text - The practice can now use SMS text messages for flu reminders & specific clinic invites.
To register for this service, simply give us your up-to-date mobile phone number.
If you have already given us your mobile phone number but would not like to be a part of this scheme, let us know and we can code this on your record so that you are not included in this system.
