What is Social Prescribing?

Social Prescribing is a free service that focuses on non-medical support for your health and wellbeing. It helps connect you with activities, opportunities, and community groups that can support you in everyday life.

Who is it for?

Anyone aged 18 or over who is registered with a GP surgery in Swansea can access this service.

How do I access Social Prescribing?

You can be referred through your GP Practice, or you can ask your GP Practice to refer you.

How does it work?

The aim of Social Prescribing is to help you take an active role in improving your wellbeing.

Once a referral is received, a Social Prescribing Coordinator will contact you to arrange an appointment. They will:

  • Listen to what matters to you
  • Discuss your needs and goals
  • Provide information about local and wider community services
  • Help identify any barriers that may be stopping you from accessing support

This is a short-term service designed to help you find longer-term support.

What support can Social Prescribing help with?

Social Prescribing can connect you with local support including: Emotional support, Social activities, Physical activity groups, Support for carers, Volunteering opportunities, Advice and assistance.

Important to know

Social Prescribing is not an emergency or crisis service and staff are not medically trained.

If you need urgent medical or mental health support, please contact your GP Practice or emergency services.
