Privacy, Medical Records and Your Rights
How we protect your information
The practice complies with data protection and access to medical records legislation. We take your privacy seriously and only share identifiable information when there is a clear reason to do so.
When your information may be shared
Identifiable information about you will only be shared with others in specific circumstances, including:
For your medical care
For example, with district nurses, hospital services or other healthcare professionals involved in your treatment.
To help you access services
For example, social services. This would usually require your consent.
When there is a duty to protect others
For example, in child protection cases or where there is a legal duty to share information.
For planning health services
Anonymous information may be used locally or nationally to help the Health Board and Welsh Government plan services.
Anonymous information does not identify you. If you do not want anonymous information about you to be used in this way, please let the practice know.
Welsh GP Record
Your Welsh GP Record is a summary of important information from your full GP medical record. It can be viewed by selected health professionals in Out of Hours services and certain secondary care providers.
What information is included?
- Your name, address and contact details
- Your current medication and medication prescribed in the last two years
- Allergies or adverse reactions
- Current problems or diagnoses
- Recent test results from the previous year, such as blood tests and x-rays
Your Welsh GP Record does not include private discussions you may have had with your GP.
Management of records
Records and information management help us make sure the right information is available at the right time to support safe and effective care.
Organisations are required to create and manage records appropriately under the UK General Data Protection Regulation, the Data Protection Act 2018 and the Freedom of Information Act 2000.
Records can include administrative records, paper and electronic documents, emails, audio and video recordings, x-rays and CCTV footage.
Subject Access Requests
You are entitled to request a copy of your medical records. To do this, please apply in writing to the Practice Manager by completing the Subject Access Request form.
Requests are usually actioned within 30 days. Depending on the volume of paper records, you may be asked to collect your copy from reception.
Apart from the doctors, practice nurses and practice staff, district nurses, health visitors and midwives attached to the practice may also have access to medical records for patients in their care.
